At Lake Highlands Acupuncture, we want to start treating you quickly so you begin feeling better as soon as possible.  To save time, please complete the intake forms before your appointment, and then:

 

 

• scan and email them to info@lakehighlandsacupuncture.com,

• drop them off at the office any time before your appointment,

• or bring them with you to your appointment.

 

 

When possible, we prefer to receive your paperwork before the appointment because it allows us ample time to study your specific case and develop a more thorough plan.  If you cannot fill out the paperwork at home, please arrive at least 15 minutes before your appointment to complete it.  We are a busy clinic, so arriving late without paperwork may mean that we need to reschedule your appointment.
